Innovaccer has acquired AI-powered platform Story Health to lead the development of AI agents that enhance speciality care teams.
The deal extends Innovaccer’s Healthcare Intelligence Cloud into agentic care augmentation and represents its latest acquisition, following Humi AI, Cured, and Pharmacy Quality Solutions.
Story Health helps to move speciality care from episodic visits to continuous digital management. The platform integrates AI-guided clinical pathways, medication workflows, and human-led coaching.

Through Story Health, Innovaccer will pioneer agentic care augmentation, with AI agents supporting specialist care teams by handling routine, non-clinical tasks and sustaining ongoing patient engagement between appointments.
Working alongside these AI agents, health coaches can speak directly with patients about symptoms and adherence to medication.

The acquisition strengthens the Healthcare Intelligence Cloud and drives scalable clinical transformation, enabling continuous specialist care supported by AI for both providers and health plans in the US.
Existing Story Health customers will continue to receive uninterrupted service, along with wider access to Innovaccer’s expanded AI-powered capabilities.
Story Health will also introduce programmes for diabetes and chronic obstructive pulmonary disease.
